David J. Usher, M.D.
Dr. Usher founded ReforMedicine, S.C., in 2011 as the Chippewa Valley’s first and only direct-pay medical practice. Since that time he has served thousands of individuals with medical care and physician-led weight loss visits. Dr. Usher has been active in the Eau Claire Area Chamber of Commerce, the Greater Menomonie Area Chamber of Commerce, the American Academy of Family Physicians, the Obesity Medicine Association, and the Catholic Medical Association. He volunteers at the Chippewa Valley Free Clinic. He is also active in his family’s activities such as scouting, school, athletic, and church events.
In 2013, Dr. Usher became recognized as a Diplomate of the American Board of Obesity Medicine. Prior to starting ReforMedicine, Dr. Usher had been practicing for 13 years with Mayo Health System in Eau Claire. His education history began at Creighton University in Omaha, Nebraska, and then continued at the University of Iowa Medical School. Once he received his M.D., he and his wife moved to North Carolina where he completed his residency at the Duke University/Fayetteville Area Health Education Center Family Medicine Residency Program. Upon graduation and passing of board exams, they moved to Kodiak, Alaska, where he spent 26 months working as a family doctor for the Kodiak Area Native Association. From 1999 to 2011 he treated patients at Mayo Clinic Health system in Eau Claire as a Family Practice Physician and served as the department chair of Family Medicine from 2006-2011. Dr. Usher was also the Medical Director for the Weight Management Services department for about seven years.
Nola Westphal, M.D.
Dr. Nola Westphal joined ReforMedicine as a Board Certified Family Medicine Physician in 2013. A graduate of the University of Minnesota-Duluth and the University of Minnesota Medical School, Dr. Westphal completed her residency at the UW Family Medicine Clinic in Eau Claire and now sees patients on a part-time basis at ReforMedicine.
Dr. Westphal is a longtime resident of the Menomonie area where she resides with her husband Jon, a local business owner, and three children, 1 away at college and 2 in high school. Dr. Westphal is involved in her children’s activities and sports, volunteering at church and school. She enjoys horseback riding, gardening, traveling, walking the dog. and spending time with family and friends.
Christina Funk, DNP, FNP-C, APNP
Christina Funk is a Board Certified Family Nurse Practitioner and joined the ReforMedicine team in the summer of 2015. She specializes in both Family Medicine and Obesity Medicine. Christina graduated from North Dakota State University with her Bachelor’s Degree in Nursing. She spent several years as a critical care nurse in a variety of settings including a medical/surgical critical care unit in Fargo, North Dakota; a general intensive care unit in Ithaca, NY; and the critical care unit at Sacred Heart Hospital in Eau Claire, WI. Christina earned her master’s and doctoral degrees at the University of Wisconsin, Eau Claire and is certified through the American Association of Nurse Practitioners (AANP). Prior to ReforMedicine, Christina has worked in Nursing Education at a local college and as a NP at Mayo Clinic Health System-Eau Claire. Christina is an active member of the Obesity Medicine Association and the AANP.
Christina lives in Eau Claire with her husband, a scientist, and their four children. She enjoys family bike rides, playing soccer, watching her children play soccer and being active. She spends time volunteering for the local soccer club, her children’s school, and has started a local chapter of Girls on the Run.
Renee Carlson, MSN, ANP-C, FNP-C, APNP
Renee Carlson joined the ReforMedicine team of clinicians in 2016 and became a full-time provider in 2017. She sees patients at the Eau Claire ReforMedicine clinic several days each week and is also stationed at an on-site clinic for the Chippewa Falls Area School District several days each week.
Renee is currently pursuing a Doctor of Nursing Practice degree (DNP) at Maryville University. Her Master of Science Degree and Bachelor of Science degree were each earned at UW-Eau Claire. She has also completed a Post Masters Emergency Medicine Certification/Fellowship with West Virginia University. Her work experience involves Emergency/Urgent Care and primary care, both in rural and urban healthcare communities. Renee is also an active member of the American Association of Nurse Practitioners (AANP).
Renee and her husband are involved with their two children’s sports and activities including volunteering at school and at church. Renee enjoys running (in the company of her dog, Jonah), gardening, spending time with family and friends, traveling, reading, knitting, cooking, and baking.
Dana is Nurse Practitioner with a Bachelor’s from Viterbo University, and Master’s from Chamberlain College of Nursing. Dana is continuing her education further at Viterbo University attending a new program with dual degrees including a Doctorate of Nursing and a Master’s in Business. Most of Dana’s entire nursing career has been at Mayo Clinic working in Family Medicine for about the last 11 years, previously in the Medical Surgical Unit. Chippewa Falls is her hometown where she grew up, went to school, and now currently resides with her four children, significant other Andy, and their merged family. Dana loves to spend free time with family and friends, reading, painting, and volunteering.
Amanda Yule, RN, Nurse Manager, joined ReforMedicine in October of 2018. She earned a Nursing Associates Degree from Chippewa Valley Technical College in 2011 and a BSN from UWEC in 2017. Amanda is currently enrolled at American Sentinel University working on a Masters Degree in Nursing Management and Organizational Leadership. Amanda is from Eleva, Wisconsin, married to Ryan, and has four children. Her free time is busy with supporting her children’s activities at school, sporting events, and church events and volunteering. Amanda enjoys spending time with family, reading, cooking, and gardening.
Samantha Hill, Certified Medical Assistant
Sam Hill joined ReforMedicine as a Certified Medical Assistant full-time in September of 2012. She is a 2011 graduate of Chippewa Valley Technical College in Eau Claire, Wisconsin and is certified through the American Association of Medical Assistants (AAMA).
Sam is originally from Durand and moved to Menomonie in 2011 where she resides with her husband Eric, and their daughters Chloe and Layla. In her spare time she enjoys being with family and friends and exploring the outdoors.
Maly Moua, Certified Medical Assistant
Maly Moua joined ReforMedicine in 2013 as an intern and transitioned to a full time Certified Medical Assistant in the spring of 2015. She received her medical assistant diploma from Chippewa Valley Technical College in Eau Claire and is certified by the American Association of Medical Assistants (AAMA). Maly greatly enjoys helping patients as well as working with the exceptional team of providers at ReforMedicine.
Maly grew up in nearby Chippewa Falls and currently calls Eau Claire home. She enjoys spending her free time with her family, enjoying outdoor fun activities and nice walks.
Jillian Allen, Certified Medical Assistant
Jillian Allen is a full time Certified Medical Assistant at ReforMedicine since January 2017 after graduation from Chippewa Valley Technical College in late 2016. Jillian is certified through the American Association of Medical Assistants (AAMA). A native of Cumberland, she moved to Eau Claire a few years ago where she lives with her fiance, Justin. In her spare time, Jillian enjoys spending time with her family, watching hockey, and being outside.
Nicole has been in the medical field since 1995 starting as a CNA, where she learned that she enjoyed helping people. After leaving the US Navy in 2003, she re-joined the medical field and since then has completed schooling to be a Medical Assistant (graduating in 2010). She is also a military spouse and has had the pleasure of working in clinics in other states such as Missouri, Minnesota, and Wisconsin. Wisconsin is her home state, and she and her family are glad to have moved back to stay. Nicole became an EMT-B in 2014, and that additional training carries over to help in the clinical setting. Nicole is from Glenwood City, and now calls the Durand area home with husband Chad and two children Leland and Raegan, and dog Oscar. In her free time she enjoys spending time with all of her family, traveling, cooking or baking, canning, watching movies, and just being on the go.
Isabella Herrem, Certified Medical Assistant
Isabella Herrem, a Certified Medical Assistant, is a 2018 graduate of CVTC’s Medical Assistant program. She joined ReforMedicine as an intern during her program at CVTC and was hired on after graduation. Isabella is a Menomonie native and graduate of Menomonie High School, and is currently continuing her education at UW-Eau Claire and working part-time at ReforMedicine. In her free time she enjoys spending time with her family and pets.
Lisa has been in the medical field since 1996, starting as a CNA. Her career included medical reception/office management roles and medical assistant and surgical tech, as well. She is a military spouse and has worked in Minnesota, Wisconsin, Mississippi, and South Carolina. She has returned to Wisconsin to be near family. Lisa and her husband, Gregg have 4 sons, 7 grandchildren, and their dog, Bella. Lisa enjoys reading, walking, snowshoeing, and spending time with family. She is active in her church and enjoys volunteering with the elderly.
Rachel Zacho, Office Manager and Director of Client Services
Rachel Zacho joined ReforMedicine in January 2017 and is the Office Manager and Director of Client Services. Prior to ReforMedicine, she was employed at LOGICARE Corporation as Office Manager for 13 years.
In her spare time she enjoys walking, gardening and life.
Michele is a native of East St. Paul, MN, and now calls Knapp, WI home. She has been working in the medical field since 2004 and is excited to join the ReforMedicine staff. In her free time she enjoys spending time with friends and family, cooking, baking, camping, hunting, fishing (just don’t ask her to take them off the hook, clean or eat them), cross stitch, and crocheting.
