The Cardinal Healthy Primary Care Clinic team is very excited to meet you and help care for you and your family!
Tom Joles, MD

Dr. Joles was born and raised in the Chippewa Valley and graduated from Chi-Hi in 1993.  After graduation, he enlisted in the U.S. Army where he was first exposed to the medical field and trained as an EMT and X-Ray technician.  After 4 years of honorable service, he returned to Wisconsin to pursue becoming a Physician.  Dr. Joles earned a Bachelor of Science through the University of Wisconsin - Eau Claire, Medical Doctorate through the Medical College of Wisconsin, and completed a Family Medicine Residency through the University of Wisconsin.  Dr. Joles has been practicing in Western Wisconsin for the past 11 years where he has consistently provided personalized and thoughtful care to all of his patients.  Dr. Joles strives to empower his patients to make educated decisions in their health care by providing insight and guidance into sometimes complex health concerns.  In his free time he enjoys spending time with his wife and 4 children.

Renee Carlson, NP
Renee Carlson is a Nurse Practitioner seeing patients at Cardinal Healthy Care. She is currently working on her Doctor of Nursing Practice (DNP) degree at Maryville University. Her Master of Science Degree and Bachelor of Science degree were each earned at UW-Eau Claire. She has also completed a Post Masters Emergency Medicine Certification/ Fellowship with West Virginia University. Her work experience involves Emergency/ Urgent Care and primary care, both in rural and urban healthcare communities. Renee is also an active member of the American Association of Nurse Practitioners (AANP)

Renee is an active volunteer at her children's school and at church. She and her husband, who is also a nurse, spend time with their two children who are active in sports and many other activities.

Renee enjoys spending time with family and friends, running (in the company of her dog, Jonah), gardening, spending time outdoors, reading, knitting, cooking, and baking.
Laura, RN
Laura has been a registered nurse for 25 years and has experience in working in oncology providing IV infusions to cancer patients and triage. Laura enjoys helping patients to understand their conditions and providing education and information for them to promote a healthy lifestyle. Education for patients is empowering and helps patients to take control of their lives. Laura has two daughters and enjoys outdoor activities including pitching horseshoes, fishing and camping.
Elissa Larson
Elissa is a Wisconsin native and the MA (Medical Assistant) for Cardinal Healthy Primary Care. She is often be the voice you hear on the phone or the first smile to greet you in the Cardinal Healthy clinic.

She attended college in Colorado and lived there for more than 7 years where she worked at a Medical Assistant in a small family practice clinic for nearly 5 years.  She recently re-located back to Wisconsin to be closer to family. 

Elissa has a daughter in high school and enjoys hiking and biking and basically any outdoor activity.  She also loves a good book or just relaxing with friends or family.
Do I need to transfer my health records?
You may wish to have a copy of your Preventive Health Care information released to Cardinal Healthy Primary Care Clinic. before your first appointment. Click here for a request form you can use to contact your prior provider.
When can we access the online patient portal?
Once an individual has had his or her initial appointment at Cardinal Healthy Primary Care Clinic, the clinic staff will set up an invitation to the secure patient portal. The secure patient portal will then allow the patient to securely log in to check test results and communicate securely with his or her provider. While a message for an appointment request can be made through this system, it is most efficient to call the office. The portal account will be created at each patient's first visit and an instruction sheet for accessing the service will be provided at that time.

I'm not on the district insurance plan, is this clinic for me?
The Cardinal Healthy Primary Care Clinic is for the Chippewa Falls Area Unified School District. The clinic is where any employee will report for pre-employment screens such as TB tests, etc. as instructed by the Human Resources Department. It is also open to all employees for the wellness program and biometrics or health risk assessment testing. It is true that most services are provided as part of the health benefits program. If you have questions, please contact the Human Resources Department of the Chippewa Falls Area Unified School District.
Help-another clinic won't transfer my medical records or test results!
If you encounter any difficulties with other medical facilities cooperating with Cardinal Healthy Primary Care, please simply contact the Human Resources Department or Cardinal Healthy directly. Between the two teams we will intervene to ensure the medical facility has the proper credential information on file to allow for communication between the two facilities.
