Bariatric Practice Pricing:

ReforMedicine provides specialty care in the area of non-surgical obesity treatment known as bariatric medicine.  Our patients have safely lost and are keeping off thousands of pounds!  The most time and labor intensive visit is the initial visit where important information is gathered and shared that will put you on a path to a healthier body weight and lifestyle.  Plan for approximately 90 minutes for this visit. 

Initial visit : $295 (a 20% savings!)

If your insurance or other third party will pay for your labs and KG to be done elsewhere, Dr. Usher can order these for you as part of the initial $295 consult fee.  Tests done within thirty days at an outside facility prior to the initial consultation may be acceptable in some circumstances as determined by Dr. Usher.  Use of these previous labs will not constitute grounds for any reduction in the initial consultation fee.

Subsequent visits with MD:

*Pre-paid plans are for visits in consecutive calendar months following the purchase.  If a visit is missed for any reason, the visit is simply lost to the purchaser. One may not pay for 3 visits and choose the months he or she wishes to come.  Pre payment purchase the services in a certain time frame e.g. 3, 6, or 12 months.  These visit purchases are NON-REFUNDABLE.

Lab and EKG

ReforMedicine Bariatrics recommends that all our patients be aware of their health risks posed by being overweight.  We offer lab services at very reasonable charges right on site.  For those who need more extensive lab analysis, we offer package pricing to make  lab work more affordable.  If you are unsure of your lab needs, please come to your appointment fasting for at least 8 hours.  Water and black coffee/unsweetened tea are OK in those 8 hours. 

If the doctor recommends and the patient chooses to use an appetite suppressant medication, Wisconsin State Law requires an EKG be done at the time of starting the medication.

Note:

Bariatric Medicine is a separate specialty service from family medical services and will be billed as such even if a patient is seen on the same day for other family medical services (for example, annual physical and obesity management = $55 + $55 = $110.)
