How Do I know if I'm a "Covered Employee?"
You may contact the Human Resources department at WESTconsin Credit Union or contact ReforMedicine directly. Most employees and their dependents have access to the direct pay membership at ReforMedicine.
I have questions-who do I call?
WESTconsin Credit Union covered employees and their beneficiaries are welcome to contact the WESTconsin Credit Union Human Resources Department or contact ReforMedicine directly. When you call and identify yourself, ReforMedicine staff can answer any questions about services available to you, costs, scheduling, or other questions you may have. Please do not send confidential information of any kind via inquiry forms or e-mail as it is not considered to be secure delivery.
How do I learn about pricing?
The office visit fee for covered employees is $10. WESTconsin Credit Union covered employees have special discounted pricing because of a corporate membership that your employer has purchased. Annual physicals are no charge. Discounted pricing and included items for members can be viewed here. If there is ever a question, please call a ReforMedicine office or check with WESTconsin Credit Union Human Resources.
How do I choose a provider?
ReforMedicine is a family practice, meaning all of the providers are primary care or certified family medicine providers. Providers include Physicians, Nurse Practitioners, and Physician Assistant who are qualified to be your primary care provider. You are welcome to set an appointment with any of our available providers when you visit. Some providers have additional specialties such as medical weight loss or certifications for DOT physicals, etc. If you have a special need please explain it when you call and your appointment will be set accordingly.
How do I set an appointment?
The best way to set an appointment is to call a ReforMedicine office by phone. You may call Eau Claire at 715-514-2827 or Menomonie at 715-895-6234. If one office is unavailable the outgoing message will direct you what to do and voicemail messages are retrieved regularly. If your need isn't urgent and e-mail contact is preferred, please use the form on the ReforMedicine website to make your request or once you have become an established patient and have access to the secure patient portal, you may use that service to message our offices about appointments. Please know that phone calls always receive priority over online requests.
How do I set up the Patient Portal?
At your first appointment you will receive instructions for setting up your secure patient portal. This service will allow you to see a summary of appointments, test results, and also provide a secure manner in which to message your provider (unencrypted e-mail and forms are not considered to be secure for messaging).
