Routine annual wellness exam, labs and vaccines

As part of your near site clinic benefit you receive your annual wellness physical/wellness exam, routine labs, and vaccines for FREE. This includes everyone on the ReforMedicine health plan (employees, spouses, dependents).

FREE Flu Shots & Immunizations

Stay well with FREE immunizations

Schedule an appointment to receive your FREE seasonal flu shot. Routine immunizations are included for free and are usually done during your annual wellness physical/exam.

$10 Office Visit Fee

Access High Quality Care Inexpensively

Any other office visit is just $10 for employees, spouses, and dependents covered ReforMedicine health plan. This could include care of chronic conditions, acute illness, non-emergency injury, or non-routine physical.

Members of the ReforMedicine health plan have access to no cost lab tests. A lab-only visit is no charge.

Priority Access

SAME- or NEXT-DAY Appointment

Plan members have priority access to PRIMARY CARE APPOINTMENTS when  there is a matter requiring prompt medical attention.

Please CALL the office if you think you need to be seen SAME- or NEXT BUSINESS-DAY.

Medical Weight Loss

$10 Startup Fee

This includes intake/welcome appointment with a Medical Assistant, Intake labs, InBody (body composition), EKG, Medical Weight Loss video, Medical Weight Loss guide book, Initial Provider visit, and access to health coaching. After the initial bundle, the patient/provider visits are $10 and Health Coach visits are $20

How to Schedule an Appointment at ReforMedicine

  • Patients may schedule by phone 715-514-2827 or via the secure Spruce app. If you don't have the Spruce app, you can text "app" to 715-514-2827 to receive an invitation.

  • SAME DAY or NEXT BUSINESS DAY Primary Care appointment requests may be made by phone or messaging via the Spruce app.

  • The secure patient portal allows established patients to make regular visit, non-urgent appointment requests online, but does not allow the patient to see available appointment times. Calling or using the Spruce app is typically a more efficient process for scheduling an appointment.

  • Scheduled telemedicine visits are available using the Spruce app on your phone or device.  When scheduling, please be aware that certain problems discussed during a telemedicine visit may prompt the provider to recommend coming in to clinic to be seen in person for further evaluation.  If you are concerned you may be too ill for a telemedicine visit, we recommend scheduling an in-person appointment.
