FREE Annual Wellness Physicals
Routine annual wellness exam, labs and vaccines
As part of your near site clinic benefit you receive your annual wellness physical/wellness exam, routine labs, and vaccines for FREE. This includes everyone on the health plan (employees, spouses, dependents).
FREE Flu Shots & Immunizations
Stay well with FREE immunizations
You can visit the ReforMedicine office to get your FREE flu shot with an appointment. Routine immunizations are included for free and are usually done during your annual wellness physical/exam.
$20 Office Visit Fee
Access High Quality Care Inexpensively
Any other office visit is just $20 for employees, spouses, and dependents covered on the health plan. This could include care of chronic conditions, acute illness, non-emergency injury, or non-routine physical.
Deeply Discounted Lab Tests
Most labs are just $15

Priority Access

Members have priority access to PRIMARY CARE APPOINTMENTS when there is a matter requiring prompt medical attention.

Medical Weight Loss
Patients Pay $200
This includes intake/welcome appointment with a Medical Assistant, Intake labs, InBody (body composition), EKG, Medical Weight Loss video, Medical Weight Loss guide book, Initial provider visit, and access to health coaching. After the initial bundle, the patient pays for the provider/office visit $20 each.

If you need an appointment SAME DAY or NEXT BUSINESS DAY, please CALL the office to ensure your need is addressed as quickly as possible.

They were able to get us in to be seen very quickly and after work hours. They examined her and decided to do bloodwork and found that she tested positive for Lyme Disease. "All members of our family have been served over the first half of 2017 at ReforMedicine, and our experience has been uniformly positive. The most impressive testimonial we can offer involves some skin procedures that Dr. Usher provided free of charge during a free preventive visit (general physical). A visit to another clinic's dermatologist late last year for the same type of skin procedure resulted in a $1,068 bill (after discounts) that we paid as part of our deductible. Those charges included $140 for the procedure itself, $311 for a biopsy, and $617 for the pathology on the biopsy. This is despite an extremely low chance that the procedure involved anything cancerous, but the dermatologist recommended the biopsy and lab work. Dr. Usher said there was no need to biopsy the similar skin procedure he carried out months later. And, to emphasize, the procedure was done at no charge.
