As part of your near site clinic benefit you receive your annual wellness physical/wellness exam, routine labs, and vaccines for FREE.

FREE Flu Shots & Immunizations

Stay well with FREE immunizations

Schedule an appointment to receive your FREE seasonal flu shot. Routine immunizations are included for free and are usually done during your wellness physical/exam.

Traditional Health Plan: Office visits fees $0.  E-Visits $0. High Deductible Health Plan: Office visit fee $25. E-Visits $0. This could include care of chronic conditions, acute illness, non-emergency injury, or non-routine physical.

Lab Tests

Low or No Cost Labs

Members of the Traditional Health Plan have access to $0 lab tests. High Deductible Health Plan (HDHP) members have $5 per lab visit.

In-Clinic Dispensed Medications

$5 Common Prescriptions

As we are not a retail pharmacy, ReforMedicine has only a limited supply of common, pre-packaged medications that we can dispense. These prescriptions are available to you at $5 per bottle. Please ask your provider about which medications are available.

Medical Weight Loss

Low or No Fee Medical Weight Loss

Traditional Health Plan: $0 startup and $0 appointments, High Deductible Health Plan: $30 ($25 for intake visit and $5 for intake labs) startup and $25 appointments. The initial start includes intake/welcome appointment with a Medical Assistant, Intake labs, InBody (body composition), EKG, Medical Weight Loss video, Medical Weight Loss guide book, Initial Provider visit, and the first five sessions of health coaching appointments. Health Coaching (beyond 5) $20 per visit.

How to Schedule an Appointment at ReforMedicine

  • All new patients must schedule by phone:  715-514-2827

  • SAME DAY or NEXT BUSINESS DAY Primary Care appointment requests MUST be made by phone.

  • The secure patient portal allows established patients to make regular visit, non-urgent appointment requests online, but does not allow the patient to see available appointment times.

  • Scheduled telemedicine visits are available using the Spruce app on your phone or device.  When scheduling, please be aware that certain problems discussed during a telemedicine visit may prompt the provider to recommend coming in to clinic to be seen in person for further evaluation.  If you are concerned you may be too ill for a telemedicine visit, we recommend scheduling an in-person appointment.
