FREE Flu Shots & Immunizations

Stay well with FREE immunizations

Schedule an appointment to receive your FREE seasonal flu shot. Routine immunizations are included for free and are usually done during your annual wellness physical/exam.

Access High Quality Care Inexpensively

Office visits cost $0 for Plan Participants. This could include care of chronic conditions, acute illness, non-emergency injury, or non-routine physical.

Deeply Discounted Lab Tests

Most labs are just $15

Members of the ReforMedicine health plan have access to deeply discounted lab tests. Check prices and availability any time by contacting ReforMedicine.

E-Visits $0

When you have a need for medical care that can be accomplished via an E-Visit with ReforMedicine, the cost is $0. Schedule these just as you would a regular appointment. You will need to have a camera-enabled computer or smartphone with internet access to participate in an E-Visit.

  • Patients schedule by phone to be seen at the Plank Onsite Clinic (or the ReforMedicine clinics) by calling ReforMedicine Eau Claire 715-514-2827

  • The On-Site Clinic at Plank is open the first Tuesday of each month from 8:00 a.m. to 12:00 p.m. and the third Tuesday of the month from 1:00 p.m. to 5:00 p.m.

  • Employees ONLY may be seen at the Plank On-Site Clinic.

How to Schedule an Appointment at ReforMedicine

  • All new patients must schedule by phone: 715-514-2827

  • SAME- or NEXT BUSINESS DAY primary care appointment requests MUST be made by PHONE.

  • The secure patient portal allows established patients to make regular visit, non-urgent appointment requests online, but does not allow the patient to see available appointment times.

  • Scheduled telemedicine visits are available using the Spruce app on your phone or device.  When scheduling, please be aware that certain problems discussed during a telemedicine visit may prompt the provider to recommend coming in to clinic to be seen in person for further evaluation.  If you are concerned you may be too ill for a telemedicine visit, we recommend scheduling an in-person appointment.
