Schedule an appointment to receive your FREE seasonal flu shot. Routine immunizations are included for free and are usually done during your wellness physical/exam.

Office visits fees Traditional Plan: $0, HDHP: $10. E-Visits Traditional Plan: $0, HDHP: $10. This could include care of chronic conditions, acute illness, non-emergency injury, or non-routine physical.

Any Lab is NO COST to you!
Members of the ReforMedicine health plan have access to FREE lab tests. Need labs for a specialist or some other appointment? Have them done at ReforMedicine for FREE!

As we are not a retail pharmacy, ReforMedicine has only a limited supply of common, pre-packaged medications that we can dispense. These prescriptions are available to you at $0. Please ask your provider about which medications are available.
Medical Weight Loss
Initial Start of the program and Monthly $0
The initial start includes intake/welcome appointment with a Medical Assistant, Intake labs, InBody (body composition), EKG, Medical Weight Loss video, Medical Weight Loss guide book, Initial Provider visit, and the first three sessions of health coaching appointments. Health Coaching (beyond 3) $20 per visit.

If you need an appointment SAME DAY or NEXT BUSINESS DAY, please CALL the office to ensure your need is addressed as quickly as possible.

ReforMedicine offices accept Mastercard, Visa, and Discover, or checks for payment of the office visit fee, lab tests, or other services you might purchase from ReforMedicine.
