ReforMedicine Family Physicians are residency trained, and board certified by the American Board of Family Medicine. All the providers at ReforMedicine have a strong belief that your care provider has a responsibility to always keep in mind the whole person when making health care decisions. ReforMedicine staff works hard to be punctual and your early arrival and availability in the exam room at the time of appointment helps keep you and our team on schedule throughout each day.

Plan to spend time with your provider.

We schedule much longer appointments than insurance-based practices and take time to listen to all your concerns. If you are in a hurry, we work to get you on your way as quickly as possible, but most patients enjoy the unhurried pace of appointments at ReforMedicine. During your appointment your provider will be able to tell you the cost of any procedure, lab test, or future visit they might recommend.

Pay for your appointment.

Pay for your appointment at the conclusion of your appointment, before you leave the office. You will receive a receipt. ReforMedicine accepts check, credit or debit card (including Health Savings Account 'HSA' and Flexible Spending Account cards).

Paperwork

ReforMedicine does not bill insurance companies, therefore, there is no insurance paperwork to fill out. As a new patient at our clinic, we will ask for some basic patient information and try to make your first visit with us as easy as possible.

Payment

You will be asked to pay for your visit the day of your visit. ReforMedicine does not bill patients afterward. ReforMedicine accepts checks, and Visa and Mastercard debit and credit cards and Discover Card. This includes HSA debit/credit cards or Flexible Spending Account debit/credit cards.

Family Practice Primary Care Appointment Pricing

Level 2 Appointment $205

A Level 2 appointment is the most common type of appointment at ReforMedicine.

  • Well child checks

  • Adult complete physicals up to age 64

  • CDL physical, insurance or other special purpose physical

  • Multiple chronic illness and condition management

  • Long-term multiple medication management

  • Level 1 problem that did not go away

  • Acute or chronic illness management

  • Review of tests, labs, imaging, or consults from another clinic

Level 1 Appointment $135

A Level 1 appointment would include an office visit to care for any single ONE of the following issues:

  • Cold/sinus or flu

  • Bladder infection (includes urinalysis if needed)

  • Sore throat (includes rapid strep test if needed)

  • Minor eye problems such as stye or pink eye

  • Ear infections

  • Impacted ear wax (including ear flush if needed)

  • Tick & insect bites (Lyme Screen available for an additional charge)

  • Acute bronchitis

  • Skin rashes (acute illness type)

  • Pregnancy testing (includes urine pregnancy test)

  • Single issue follow up from a Level 2 visit (routine Rx refills, re-check labs, etc.)

  • Middle School or High School Student Sports Physicals

Level 3 Appointment $265

For all patients NEW to ReforMedicine that are aged 50 or over, the initial new patient visit charge will be $265. Once established, future appointments will follow the standard level charges. New Patients are defined as patients who have not been seen at any ReforMedicine locations for a provider visit within the last 36 months.

An adult physical for ages 65 and over, new or established, is a Level 3 visit.

*Patients Under 18 MUST have a provider's order for lab testing. Patients under 18 are encouraged to meet with a ReforMedicine provider for an appointment to determine lab needs at the provider's discretion, OR an outside provider's order may be faxed to the clinic.

Appointments

Appointment scheduling is best accomplished by calling or messaging with us via the Spruce app.
Online scheduling requests can be made through the secure patient portal for returning patients.

Fee-For-Service Policies

If you should fail to contact ReforMedicine S.C. by phone or Spruce message to cancel your appointment no less than 30 minutes prior to your appointment start time, you will be charged the current Level 1 appointment cost for each 30 minute appointment period for which you were scheduled. This amount will be due and owing on the date of the appointment that you failed to cancel. No further appointments will be scheduled for you until the Failure to Cancel fee is paid in full.

Failure to pay your entire bill at the time of service will incur a $50 accounts receivable management fee and future office visits will be required to be pre-paid (credit/debit card only) before another appointment will be scheduled. For each business day (including Saturdays) the outstanding service bill is owed, an additional fee of 5% of the current listed Level 2 appointment price will also be charged for accounts receivable management.

Writers of returned checks ("bounced checks") will be charged a $50 processing fee in addition to the services owed, and an additional fee of 5% of the current listed Level 2 appointment price for each business day (including Saturdays) the outstanding service bill is owed until an alternative form of payment (card) is received.

Medicare Patient Policy
Medicare/Medical Assistance/Badgercare patients, by law, may not seek reimbursement from insurance for care provided at ReforMedicine, S.C.
