Wellness Care
Primary care providers care for patients of all ages from newborns to centenarians providing wellness care. Well child checks, Middle school and high school sports physicals, and adult complete physicals are all provided at ReforMedicine.
Acute Care

If you or your provider are concerned about snoring, poor sleep, or possible sleep apnea, ReforMedicine offers at-home sleep studies as a convenient alternative to overnight lab testing. Instead of spending the night in a hospital or sleep center, you can complete your study in the comfort of your own bed using a small, easy-to-use device.
How It Works

Provider Orders Your Test – Your ReforMedicine provider will decide if a home sleep study is right for you.

Device Shipped to Your Home – A small, easy-to-use sleep monitor arrives at your door with clear instructions.

Two Nights of Testing – Simply wear the device at home while you sleep in your own bed.

Return the Device – Send the device back using the included return packaging as instructed.

ReforMedicine will be partnering with mobile imaging services to provide scheduled on-site portable direct-pay imaging services for Fee For Service patients.
